1. Fill out the New
Patient Questionnaire as completely as possible,
print it out and send or fax
it in. Do not return by email.
2. Along with the printed questionnaire, send copies
of the following items, if they have been done:
- radiologist typed reports of recent scans, X-rays,
etc. (Do not send the films themselvesjust the
reports)
- recent lab tests
- pathology reports from biopsies
- Physician Consultation Reports from any specialists
whom you have seen for first and second opinions
- A list of all current medications, vitamins, herbal
remedies, nutritional supplements, etc.
3. Make your preliminary payment by check or
credit card for $400. If you wish to pay by credit card,
please call Hill Park Medical Center at (707) 778-3171 and they
will accept your payment. This payment covers the initial
review of your records and our first telephone meeting
which lasts one to one and a half hours. Conferencing
can be arranged to allow other family members or friends
to participate.
4. Please call IMS at
707-773-0333 to answer any questions you may have and
set up your first appointment.

During your first telephone appointment with Dr.
Bouch, you will be asking each other a lot of
questions. Dr. Bouch will want to know about your condition
and decisions that have been made about your care. You
will want to share your concerns with him and ask about
your options. Based on that conversation, Dr. Bouch
will share his preliminary thoughts with you and together
you will determine the next steps. In most cases, Dr.
Bouch will then:
- review your medical records
- launch a global, medical literature search to discover
articles, treatments, and clinical trials related
to your condition
- analyze all of the information and share his findings
with you in subsequent phone appointments
- work with you to formulate a personalized plan that
offers the best opportunities for your improvement
- maintain regular phone contact with you to keep
the process on track, chart your progress, and discuss
any new ideas or concerns you may have
